How Can Side Effects Of Long Term Use Of Mycophenolate Be Managed?

Question: Have been on Mycophenolate 500mg for last 6 months cause steroids no longer effective after 50 years of use, now 65 years old. Last 6-8 weeks have lost my appetite, much flatus, belching, constipation, 8 pounds of weight loss. Last 5 days have decreased mycophenolate to once a day and believe my symptoms are better but still present. Have had nephrotic syndrome, urine negative for protien at this juncture.

Hi,

Mycopenolate is pretty effective drug, however the problems you said can all be caused by the drug, and these are not uncommon with the this drug. It has been always pretty hard on the stomach, can cause cause low hunger, vomiting, bad taste, dumping, gastritis, constipation, along many other possible (and many more serious like hepatitis, pancreatitis) side effects which we are not concerned here at the moment. it can also cause weight loss.

So what I can see is that probably its this drug that have caused, especially if you say that the problem has reduced with reduction of the dose.

What I think we should do now is, I would like you should consult your own doctor and and discuss the issue, he would also see for other possible effects of the drug and may run some tests as well. You can discuss with him if any alternative medicine (there are many other that may work for you) can be prescribed for your disease if this drug is hurting you a lot. However, you should not make changes in your drugs without your doctor consultation until really needed.

Meeting your doctor is necessary to see you in more details and plan you ahead, as very long history of steroids use, and on mycophenolate can cause gastritis and peptic ulcer disease which may also give such symptoms, so he may need to give you some acid suppressing drugs, or even plan an OGD (That is to view your stomach with a camera scope) if needed.

We do not say there cannot be any other cause for these symptoms, but at the moment the relation with the drugs it make more likely that these are caused by the drug.

Till that you can use more fluid, avoid food that can cause bloating like sweets juices, apple, onion and mixed food like pickle, and processed food. You can also use some OTC Antacids for relief at the time being.
