Question: Have I had a miscarriage?

LMP - 23/09/2018
Last cycle - 34 days
IUD removed July 2018
Pos prev test - 21/10/18
Brown spotting - 20 - 23/10/19
HCG - 730 29/10/18
Hcg - 1800 2/11/18
HCG - 9900 12/11/18
US - 12/11/18 means sac diameter 12.9mm
Gestation sac containing a yolk sac and foetal node but no heartbeat. Corpus luteum on right side.

What does this all mean? Calculations from
LMP say I should have been 7+1 when I went for my scan but the radiologist said I was more like 5 and said it wasn’t even worth doing an internal ultrasound. I need closure!

Brief Answer:
You need to repeat ultrasound can in a week

Detailed Answer:
Hello and welcome to Ask A Doctor service.

I have reviewed your query and here is my advice.

No it does not mean abortion. It means unclear report as of yet and you need a repeat scan in 1 week to assess the development of fetus. HCG is fine for gestational age. Sac size is also normal. There is a possibility that heart beat will develop in few more days.

Meanwhile take adequate rest so that repeat "spotting" does not happen.

Hope this helps. Let me know if I ca assist you further.

Based on the dates provided how many weeks pregnant do you think I actually am? Also, how many weeks does a 12.9mm mean gestational sac correspond with? How accurate are these measurements. Do you normally see a heartbeat with a sac of this size and a foetal nose present?

Brief Answer:
Heart beat usually seen after 8 weeks

Detailed Answer:
Hi again

The formula to calculate gestation age from sac size during early pregnancy is:
Mean sac diameter (in mm) + 30 = gestational age +/- 4 days.

So if sac size is 12.9 mm then gestational age is around 6 weeks, 2 days +/- 4 days as per 12/11/18 US.
These measurements are not 100% accurate. There is always some degree of deviation.

Embryo is visible in a gestational sac with Sac size of 16 mm by transvaginal scan or 25 mm by transabdominal scan. Heart beat is mostly visible after 8 weeks on transvaginal scan.
