Question: Good day. i would like an understand of theses results. they are in order they were taken. There is about a 2 week time line. is this a progressive stroke? i understad without all information it is difficult to diagnosis. Symptoms were severe headache visiual disturbances, ataxia.

I have read your query and the uploaded CT scan and MRI report ms.

The CT scan and MRI findings are consistent within the time period examinations and have not progressed.

These findings are not typical for stroke, in your case, since there is anamnesis for Lyme disease, cerebral involvement should be taken in consideration.

Vasculitis also should be considered as well, and a possible underlying cause should be evaluated

Symptoms you describe may indicate TIA, but, above mentioned conditions should have excluded first.

Lime disease was listed as a Differential and somehow received top billing on many scans. All testing for Lyme disease was negative. With that being said Do you still vasculities as a differential? patient has also had two brothers with a very similar findings on MRI%E2%80%99s. same ohysical symptoms also. brothers are deceased.
patient still continues to have minor repeated episodes of the same type. No new findings on scans. PET scan slight glucose uptake on left frontal lobe. MRA normal, MRI over the next few years essentially shows no change other than progressive small vessel disease consistent with age of patient and Progressive sinus disease.

ANCA-normal. elevated d-dimer, slightly low Red blood cells.

If Lyme disease was suspected, then testing with enzyme immunoassay (EIA) evaluation for total Lyme titer or IgG and IgM titers is necessary to evaluate this possibility correctly.

Western blot testing also may be necessary if EIA is positive.

If these tests are negative, Lyme is largely excluded.

These scan and MRI findings may indicate encephalitis, vasculitis, etc.

For a correct diagnosis, biopsy (stereotactic biopsy) and subsequent histopathological examination may be necessary.
