Question: Could an mri not show a torn labrum? Even if you got XXXXXXX injected? But the Obrien test is postive an i still have shoulder pain

Greetings. You could be even having a inflammation of one or more small sacs of fluid in the shoulder, a painful condition. Applying Ice Cold Compress / towel dipped in hot water and apply over the area after squeezing water from towel along with pain medication suggested by your doctor could relieve of your pain. I have gone through your query and understand your concern. The sensitive O’Brien test is positive in two pathologies-
1. A.C. (acromioclavicular) joint pathology
2. Labral tear
So you may be having A.C. joint pathology with may be normal labrum, as suggested by MRI.To clear the dilemma a shoulder arthroscopy along with detail clinical examination may help in finding out the source of pain.
A simple test is to inject a local anaesthesia (drug lignocaine) at the A.C. joint to see the effect.

My suggestion for you would be to have injection therapy with drugs like lignocaine and methyl prednisolone .These injection serve two purpose that is diagnostic and therapeutic. These are given intra articular that is inside the shoulder joint at specific site by orthopaedic surgeon.
