Brief Answer: Siddha or Homeopathy will not be useful Detailed Answer: Hi Welcome to Healthcare-Magic Greetings of the day The success rate of Kasai porto-enterostomy depends when the procedure is performed. If done before 70 days of age, the success rate are high. Presence of water in abdomen(peritoneal cavity) is known as ascitis. Presence of ascitis is a omnious (bad sign) denoting decompensated Liver function. Also from the reports provided the following abnormality are seen. 1. Very much Raised Bilirubin: It indicates the Kasai Enterostomy is not functioning. Bilirubin present in Liver is not getting excreted into the intestine. Present Bilirubin level is 9.47mg%, if there has been increase in the value compared to previous value, it indicates progressive Liver failure. 2. All the liver enzymes are markedly elevated: Alkaline phosphatase, GGT, SGOT, SGPT are markedly elevated 3. Low Serum Protein: Albumin is synthesized in Liver, decreased level of which indicates decreased production by Liver. Further indicating Liver failure. My Sincere and straight forward advise is do not spend precious time on alternate treatment like Siddha or Homeopathy. Being a Pedaitric Surgeon , I have operated several cases of Biliary atresia. In view of deteriorating Liver function and presence of ascitis, Liver transplant is only option. Do get back to me if you need any further assistance, will be glad to assist you. Brief Answer: Will require ascitis tapping Detailed Answer: Hi Dear Sir Being a Parent myself, I understand your concern and anxiety. The water in the abdomen is due to failing liver functions. Gross distension of abdomen presses on the chest and prevents effective expansion of lungs so causing shallow breathing sometimes. For time being ascitis tapping and removal of fluid has to be done to prevent respiratory embarrassment. Many of the patients (approximately 1/3rd of children) who responded after kauai operation may progress to liver failure as in the case of your son. Do discuss ascitis tapping with your Concerned Pediatric Surgeon. I would also suggest Christian Medical College, Vellore where concession can be given for liver transplant. XXXXXXX Hrudayala, Bangalore also do Pediatric Liver transplant. Do let me know for any further help. Take care Regards Deepak
