Question: Hello Doctor,

My question regarding STDs. I had some sex encounters 17 months back. Probably around 10 encounters in span of 8 months. All were protected vaginal sex and protected oral. Its all female partners. I tested for STDs including HIV. Chlamydia, Gonnorhea , Syphilis(RPR), Hepatitis B(Ag and Ab), Anti-HCV, Herpes IgM. All were negative. I took 7 tests for HIV in last 10 months. Please find the dates below

Last protected encounter July 2015

1. First test, Alere hiv combo test(rapid test) in 23 Feb 2016 - Result -Negative
2. Insti rapid test in Apr 2016 - Result Negative
3. Third test Alere hiv combo in May 2016 - Result Negative
4. Fourth test Alere hiv combo test(rapid test) in July 2016 - Result Negative
5. Fifth test Abbott Architect 4th gen CMIA test in August 2016- Result Negative
6. Sixth test Multiplex PCR RNA (HIV1/2, HBV,HCV) - Result Negative
7. Seventh test Abbott Architect 4th gen CMIA test in August 2016-Result Negative

Kindly advice if i need to test for any other STDs. I am concerned if any rare strain is not picked up. Kindly advice. I was taking Medrol(tapering dose) for three weeks in Feb 2016 for my skin problem.

Brief Answer:
You are free from HIV 1 and 2 infection. Need no further testing.

Detailed Answer:
Hi,
Welcome to HCM.
Thanks for posting your query.
You had protected vaginal and oral sex throughout this span of time. So the possibility of HIV/STD is quite unlikely.
A negative antibody detection test, especially a COMBO test, 3 months after the exposure itself ruled out the possibility of HIV up to the extent of 99%. More than one negative test with different types of test ruled out HIV to the extent of 100%. You had multiple negative tests. HIV can not lead to sero-conversion beyond 6 months even if the Medrol tablets have any influence on the same (you had a test 6 months after February). Moreover RNA PCR would not be affected by Tab. Medrol as it is antigen detecting test, not an antibody detecting test. So it is conclusive that you are free from HIV infection beyond any doubt. There is no such special strain which can not be detected by these tests.
Regarding STDs, you had not mentioned how long after the exposure you had your tests.

I did the test after 10 months of my last exposure. I have swollen tongue with teeth marks, Gas problem(Flatulence), Snoring, 95 mg/dl fasting sugar, Feeling like I am having lipodystrophy. Lost weight around 4 kg in one year. Mild groin pain that is recurrent(pain relieves by itself in 1 or 2 days). Not sure what else I have to test for. I did complete health check up in March 2016. All were normal.
Taking librax, VSL for Irritable bowel. Got some releif

Brief Answer:
Your symptoms are not related to HIV/STD.

Detailed Answer:
Hi,
welcome back.
Nonreactive STD tests done after 10 months after the exposure is very much conclusive and you are absolutely free from HIV and STD. All your symptoms are not at all related to HIV/STD. Follow the advice of your physician and get well soon.

Brief Answer:
Important STDs had been ruled out

Detailed Answer:
Hi,
Welcome back.
All the STDs are not having separate blood tests. Some can be tested with smear examination and some can be detected with culture and sensitivity test, some others can be diagnosed only by careful examination and experience. Genital warts, pediculosis are not having any specific investigations.
You had ruled out important STDs. Not to worry about others. Only when there is any symptoms we will look into the same.
Dr S Murugan.
