Question: Can I travel to US for a short trip

I was told by Dr.Ananthapadmanabhan that I can undertake the short trip for just a fortnight considering that I am not having any problems during morning walks and other normal activities . I walk about 5 kilometers everyday.. It used to be more earlier. Reduced now because of knee problem

Brief Answer:
Yes, you may travel

Detailed Answer:

Hi,

Yes, you may travel to US and should be uneventful because of good functional capacity. It would be more assuring if your ejection fraction is good in your echo, so please let me know your recent ECHO report. I have seen your ECG, there are some old changes and supraventricular ectopics which is fine to have.
Carry all your medicines for entire duration of stay and emergency medicines like Sorbitrate 5 mg.

Hope this helps you and get back if you have any doubts.

How important is ECHO report?? I do not have one .

I need to leave on 30th morning which leaves me with just two days. If you feel it is very important, I can get it done tomorrow and send the report to you .

Otherwise your reply is very reassuring. I was particularly worried about the term 'supraventricular ectopics'. Now what you say is identical to what my physician at the Hospital said and in fact the same words he used. Thank you.

Brief Answer:
ECHO is not absolutely necessary

Detailed Answer:

Hi,

Echo is not absolutely necessary, but it's better to have one so that we come to know the current status of heart. But I guess, you must have had some echo in last 1-2 years, and it will also do. So get it done if you can get it easily, otherwise, it's fine.
Also, supraventricular ectopics are not worrisome, but you should get one HOLTER done after you come back from US to see the presence of atrial fibrillation, after which you may need some treatment modifications.

Overall, nothing to worry and your travel should be fine. All the best.

As suggested by you, I have got ECHO done this morning. Kindly have a look and advise whether it is absolutely safe to undertake a month's travel to US . Also kindly advise any other precautions to be taken before journey and during the stay abroad.

Brief Answer:
ECHO is good

Detailed Answer:

Hi,

Your echo is fine with good heart function. There should not be any problem with your travel.

Just keep all medicines with you in a handbag along with emergency medicines like Sorbitrate 5 mg for sublingual usage along with all routine medicines. Keep your diet healthy, low salt diet. Avoid too many physical activities other than what you do routinely. Carry portable electronic bp apparatus with you if you have it.

Have a happy and safe journey.

Thanks for your clear replies. I for got to mention that I am a diabetic, but under control with Glycomet SR 500 b.d

There is a lot of controversy over statins stating that they cause elevation of blood sugar and muscular pain . Can I have your views?

I was prescribed rosuvastatin 20 mg daily and on my own switched to 10 mg daily. Was that a mistake? (I retired as Senior General Manager- Training , gsk and half knowledge is sometimes dangerous !) Now I am on rosuvastatin 20 daily.

The following are the results of recently conducted test. Are they too high? After getting these results, I am very strict with my diet. I hope they will make a better reading next time.

TOTAL CHOLESTEROL 229 mg/dl 125-200
HDL CHOLESTEROL - 27 mg/dl 35-80
TRIGLYCERIDES PHOTOMETRY 254 mg/dl 25-200
LDL CHOLESTEROL - DIRECT PHOTOMETRY 145 mg/dl 85-130
TC/ HDL CHOLESTEROL RATIO CALCULATED 8.4 Ratio 3 - 5
LDL / HDL RATIO CALCULATED 5.3 Ratio 1.5-3.5
VLDL CHOLESTEROL CALCULATED 50.8 mg/dl 5 - 40
NON-HDL CHOLESTEROL CALCULATED 201.7 mg/dl < 160

Brief Answer:
They are elevated but not too high to worry

Detailed Answer:

hI,

The lipid profile is deranged to some extent, and our target is to keep it under control in order to prevent future complications. In your case, we want total cholesterol below 200 and LDL preferably below 100, and triglycerides below 200 are fine.

There is some concern regarding elevated blood sugar and muscle pain but benefit of having these are much more than the associated risk so they are unequivocally recommended. And you'll require 20 mg to control your lipids. I guess you were on 10 mg when this profile was done and if yes then 20 mg should help or else we'll have to add some other molecule.

Also, STATINs has multiple beneficial effects other than lipids lowering, for which they are continued indefinitely even if lipids are well controlled. So you should continue with 20 mg only.

Regards
