Question: HI, I am hiv negative( i think, no tests until today)and my husband is hiv positive and on art from five yrs. Its been 8months we got married. His viral load is sometimes undetectable and it's normally 0-40. We dont drink or smoke. I eat very healthy. I am trying to conceive from past 4months. To maintain my legal status here and because of my financial restrictions I can't consult a doc here. I am planning to go to my home country (india) immediately after I get a positive pregnancy result to consult gynecologist and for hiv tests. Any suggestions for me as I am worried that incase if I conceive chances of passing hiv infection to baby as I don't know I am infected with hiv or not and my husband is hiv+.

You are a discordant couple. Your husband is having undetectable viral load due to his ART. So the chances of getting infection from your husband is less. Even then, protected exposure between you is advisable. If you want to have child, then you have to opt for assisted reproductive techniques like intra uterine insemination with seminal wash and not to have an unprotected intercourse. If you had unprotected exposures, you have to get examined frequently with HIV test. Pre exposure prophylaxis with Truvada (TNF+3TC) is suggested by somebody in case of unprotected exposures. But assisted reproductive technique is always safe.

Thanks for the advice. i need to explain my situation a little more.
!. We did Unprotected intercourse many times in the last 8months and also on my Last 4,5 months of Ovulations time to get conceived.
2. how much it costs for IUI in U.S and appreciate if you can provide the cost of IUI in my home country i.e XXXXXXX ( if IUI available in INDIA)
3. As we had unprotected intercourse many times before.. incase i got that virus from my husband still can i go with IUI?
4. We tried this month for the baby without any medications like TRUVADA, incase if i conceive, is it safe to keep the baby with any medications, For this what should i do immediately?

Hi,
Welcome back.
1. Get tested for HIV 12 weeks after your last contact. After getting a negative test result, stick to only protected contact. During the time of fertility period you can have unprotected intercourse with Pre exposure prophylaxis with the guidance of an expert, provided if you remain uninfected.
2. I don't have any idea about the cost of IUI in US, whereas IUI is available in XXXXXXX at every city, even at towns. The cost of investigations and IUI procedures put together not over than 10-15 thousands of XXXXXXX Rupees.
3. If you got already infected, after starting ART and got the undetectable viral load, you can get conceived without IUI (unprotected intercourse at the time of fertility) and continue the ART throughout the pregnancy, or for ever as per the advice of your physician.
4. If already conceived start ART by 16 weeks on wards and continue the same for ever or as per the guidance of your ART physician. You can save the baby from infected with HIV.
So you have to consult a HIV physician or an infectious disease specialist as early as possible and get his/her guidance for further steps.
