Question: Hello Doctor,
I had sex with prostitute (Safe sex only-used Condom).
Before sex I was touching her private part(Don't remember which finger i used).I did not do fingering (I think only if she cums then it means fingering was done,As far as I remember there was no fluid from her private part and I had inserted any of my finger completely).

The problem is I had PEELED/BROKEN skin (due to cold weather) with paper cut in my middle finger.I was just caressing her private part.As I dont remember which finger I used,is there possibility for HIV transmission if I had used Middle finger ?

After coming back home I washed my hands,at that time in my middle finger there was small bleeding (But only after washing,not before that).
Please help me and I am confused totally due to this.The incident happened on 48 hours before I wrote this e-mail

First of all you had protected exposure. That sex worker would not be necessarily a HIV positive individual.

During caressing the female genitals, thumb and index fingers would take part mostly than the middle finger so your transmission risk is negligible.

Transmission of HIV through casual contact. As per CDC, Atlanta, USA, sexual activities other than anal, vaginal and oral sex, all others carry a negligible risk for HIV transmission.

Thanks for the reply.
You mean even if I had used middle finger HIV virus can not enter through the Peeled skin with Paper cut finger ?
As far as I remember there was no fluid from that woman.My worry is that If by any chance if fluid had leaked in small amount and if I had not noticed it that would cause problem ?

Do I need to do HIV test ?

Hi,

I cannot say that if an external wound comes in contact with the infectious materials, you are absolutely safe. Theoretically, a possibility of HIV transmission is there. But practically, we do not come across such incidences in my practice of 30 years with HIV. I have also not heard in any conferences or from colleagues about such experiences. There might be sporadic reporting of cases elsewhere in the world.

Ruling out HIV with test will relieve unnecessary mental tension. Otherwise, small ailments in the future you will likely correlate with this incidence and worry yourself.

Wish u happy New year and Thank you very much for the reply.It will be very much helpful if you could answer my below queries separately.I will be relaxed then

1.I was caressing the private part of that Woman and did not insert my finger. In this case Vaginal fluid of that woman enter my blood ?

2.Will the Vaginal fluid come in contact while I was just caressing and did not insert Finger?

3.If there was no fluid in my finger then I am free of Transmission Doctor?

4.If I had touched fluid unknowingly while caressing Virus can enter into my blood via my Peeled skin with Paper cut ?

5.Can I believe in test after four weeks ?

1.If that lady has HIV infection and if her vaginal secretion enters into your blood through finger wound then there is every possibility of transmission of HIV infection.
2. If the woman has excited and if her secretions are profuse then a possibility of contact with vaginal fluid is there while caressing the external genitals.
3. Minimal wetness might unlikely be felt. That didn't mean that you had not come in contact with the same.
4. Possibility cannot be ruled out.
5. Certain HIV tests 4 weeks after the exposure can rule out HIV-1 infection. Rare possibility of HIV-2 infection can be ruled out by 12 weeks.

Hello Doctor,
Regarding point 4 "Possibility can not be ruled out" .I am unable to understand this statement. Could you please elaborate.

My Paper cut was similar to the image shown in https://en.wikipedia.org/wiki/Paper_cut ALONG WITH peeled skin.This cut was little below from the finger.

Can Virus enter via such Paper cut if I had touched the fluid unknowingly ??Her secretion was not profuse as far as i remember.If it had been Profuse I believe I would have felt the liquid.I want to do a check asap.Can I do it after two weeks of exposure (just for my initial confidence)?However I am planning to do after 4 and 12 weeks of exposure as well.

Hi,

HIV is a virus, very small than bacteria and can be visualized only through an electron microscope, not with the ordinary light microscope. It can enter through invisible breach in the skin. Just a contact is sufficient. At the same time it can't survive outside the body fluids for not more than a few seconds.

HIV test will be reliable only after the window period.

I am living in XXXX currently and please let me know the Name of the tests I need to do.

As you mentioned I will do test after 4 and 12 weeks.

However in Google I saw that HIV RNA test can find virus exist in my blood or not within 9-11 days.Shall I consider HIV RNA test?

Hi,

HIV Qualitative RNA PCR test or DNA PCR test can detect HIV infection as early as 7 days, but considered are conclusive by 3-4 weeks. It is a very reliable test. But it can detect only HIV-1 infection. If you want to rule out HIV-2 infection, you need a separate test. HIV-1 is common to the extent of more than 99% throughout the world except in certain parts of Africa.

Hello Doctor,
I have decided to do PCR RNA/DNA HIV test and at the same time I understood that test after 3-4 weeks and 12 weeks is necessary to come to conclusion.

Exposure happened on 29th Dec. In XXXX, they are telling to do this test 11 days after exposure.

You have mentioned this can be done after 7 days of exposure.I also want to do asap.Please confirm whether doing test on 6th XXXXXXX makes sense or not.

I have two more dates 7th and 11th XXXXXXX Please let me know which date I can do this test among these three dates (6,7,11)

I will advice you to have the HIV PCR test 3 weeks after the exposure. If the test came nonreactive 7-12 days after the exposure, it will still linger in your mind till you a get a conclusive report. It is preferable to go for the test 3 weeks after the exposure and have one more antibody detection test 12 weeks after the exposure. If you take up the test earlier than this, it would be a waste of time and money.
