Posted on Tue, 12 Jan 2016
Question: I have another heart question. I have had a few more heart palps this week than normal. But nothing horrible. About 2 big ones a day for the last few days. Anyway i took a fluconazole for a yeast infection and now i am very nervous it will cause an arrythmia! (I know should have thought of this before i took it.). Can 1 150mg dose cause a ventricular arrythmia?

Detailed Answer:
Hello!

Welcome back on HCM!

I understand your concern and would explain that fluconazole can rarely cause cardiac arrhythmia (QT prolongation and torsade de pointes) in higher doses.

The frequency of this adverse effect related to fluconazole has been less than 1% especially in patients which suffer from a cardiac conduction disorder or in elderly patients.

How long did your therapy last? Is this the total dose of fluconazole taken is some days or just an unic dose taken once?

Fluconazole can be taken in different doses (100 to 400mg), continued for weeks, without showing any adverse effects such as cardiac arrhythmia.

Have you suffered from any cardiac disorder before?

The palpitations that you have experienced could also be related to anxiety.

So relax and do not worry about.

If your palpitations persist, or even increase in frequency during the day and are associated to other symptoms like: shortness of breath, dizziness, etc. I recommend performing a resting ECG, to examine your heart rhythm.

An ambulatory 24-48 hours ECG monitoring may be needed to examine your heart rhythm trends during the day, to exclude cardiac arrhythmia may be necessary in case of persistent frequent palpitations during the next days.

Hope you will find this answer helpful!

If you have any other questions, don't hesitate to ask me.

Kind regards,

Dr. Iliri
