Advised Sinus Behind Patella, In Need Of Operation. Had Knee Replacement Surgery Followed By Skin Grafting. Curable?

Hi,

My father had a knee replacement and the surgery went fine initially but then he had occasional trouble with the skin and the doctors did grafting. But now even after 2 years he is having complication and he is now advised that there is a sinus behind the patella and that needs to be operated. Really confused about this now. Can you advice whether this happens and is that curable at all? Case history is as follows:
• Knee replacement done on 25.11.2011.
• Wound dehiscence right knee-wound debridement+skin flap+skin grafting done on 20.1.2012.
• Non-healing wound below the right knee, skin grafting was done on 13.03.2012.
• Right knee stiffness MUA right knee was done on 25.07.2012.
• Wound debridement and secondary suturing was done on 04.03.2013.> Medicine- Cetil 500mg & Zanocin 200mg for 10 days
• Stitch removed on 16.03.2013 with the advice of dressing on alternate days.

Earlier Status

• Something oozing out of the central portion of the right knee.
• After continuous dressing, oozing out from the central position stopped.
• But accumulation had started below the right knee after dressing it was minimized for a lessor period but now day by day the accumulation had increased.
• The accumulation was removed manually and the discharge was sent for culture and sensitivity test.
• Culture shows growth and according to sensitivity report taken Mikacin 1gm i.v for 7 days. w.e.f. 29.5.13.
• At the same time Lizolid 600 and Zanocin 400 taken twice daily for 2 weeks.
• Blood test was done and the report:
1.Urea-6.23 2. Sugar (F)-132 3. C-Reactive Protein(Quantitative)-28.5 4.
TSH- 1.71 5. 25(OH) Vitamin D3- 15.1


• Presently taking D-Shine (60K) once in a week, Idrofos 150 once in a month with effect from 3.8.2013 and New Bona Plus- 1 cap once daily w.e.f. July 2013.

Present Status-

Again oozing out from the central portion of the right knee from 12.8.2013. Started
dressing with Normal Saline,Betadine and applying t-bact ointment in the area. (pic
of the portion of the knee is attached)

Past History

• Some thirty years back in the leg after an injury, a sinus was formed and C&S showed a growth and based on that Dr. had prescribed medicines and the same healed.

I have studied your case.

Sinus from knee suggest infection .
best solution for infection is debridment and removal of cause .
So I will advise you to go for surgery .
YOU can do some investigation before surgery like CRP,ESR,CBC.
