What Does An Accentuated Bronchovascular Markings In A Chest X-ray Indicate?

Question:
Dr. XXXXXXX Bhavsar's answer to my below question is pasted below. My new question now as a follow up: Could the XRay reference to "Prominent central hilar vascular shadows with accentuated basal lung markings" be indicative of Lung Cancer?

The original Question and answer are pasted below:
my sister is 51 years old and she is a heavy smoker. she suffered lately difficulty of breath when laid on her back and that was associated at first with some pain in the left jaw , arm and chest. her heart rate was 60 PBM and BP was as slow as 100/70 though she used to be a hypertensive.
Echo did not indicate any abnormality except for a mild Aortic Sclerosis and then the Chest X ray gave the following noted:
- Prominent central hilar vascular shadows with accentuated basal lung markings







- Dilated unfolded aorta with prominent aortic knuckle







is this an indicated of serious condition?

I can understand your concern.
Since her echo is normal, no need to worry for major heart diseases.
Smokers are at risk of bronchitis. And bronchitis causes similar kind of x ray picture.
So better to consult pulmonologist and get done
1. Clinical examination of respiratory system
2. PFT (Pulmonary Function Test).
PFT is must for the diagnosis of bronchitis. It will also tell you about severity of the disease and treatment of bronchitis is based on severity only.
Inhaled bronchodilators and inhaled corticosteroid (ICS) are the mainstay of treatment.
Smoking cessation should be done as soon as possible. With out this, no point in giving treatment.
So consult pulmonologist and discuss all these.

Possibility of lung cancer is very low.

I can understand your concern.
This x ray picture is less suggestive of lung cancer.
Lung cancer usually appears as nodule or mass on chest x ray.
But chest x ray is not always 100% confirmatory to rule out lung cancer.
For this CT scan of thorax is needed.
So if you want to rule out lung cancer 100% then get done CT thorax.
If CT thorax is normal then no need to worry for lung cancer.

I have gone through the CT report you have attached.
No need to worry for this CT findings.
She is having subcentimeter size nodule and multiple small lymphnodes. These are common in smokers.
So at present, no need to worry much. Get done follow up CT after 6 months.
If size of the nodule is same then nothing should be done.
Smoking cessation should be done first.

Thx Dr. She saw today a plumnogist and his main concern is that all lympho nodes are inflamated and that thyroid enlarged suze is pressing against the air path and narrowing it. He gave her antibiotics and bronchities enhancing medicine and want her to redo the CT after 10 days!!! I thought CT cannot be repeated that frequent due to the induced radiation! Pls advise

Yes, you are right about radiation injury.
CT scan causes highest radiation exposure.
So better not to get done repeat CT within 10 days of time.
And no need to worry for enlarged thyroid and it's tracheal compression.
Report clearly mentioned that there is slight deviation of trachea. This can be seen in normal individuals without any symptoms.

She had thyroid function tests many times and their hormones were always on the normal range.
Symptoms started abt 2weeks ago
I have also to mention that she has 16 cats living with her in her apartment

In my opinion, no need to worry for enlarged thyroid gland. This is not a reason for her cough.
She is smoker and living with cats. So possibility of allergic bronchitis is more.

she will do the test. but if proved correct ... is it drastic? or bronchities medicine and antibiotics can cure it.I meant if PFT and other analyses prove her to have COPD, is that illness life threatening.

Yes, PFT well definitely diagnose the COPD if at all present. It will also tell you about severity of the disease.
So get done PFT first.
COPD is chronic, progressive inflammatory disease of lungs. If not treated properly, it can be life threatening. So prompt diagnosis and appropriate treatment is must for the better prognosis of COPD.
So get done PFT and etc me know the result of PFT.
