What Causes Increase In Breast Size During The Fifth Month Of Pregnancy?

Question: Hello doctor,
My wife is in her second trimester of pregnancy.She would be entering the 5th month of pregnancy.
I wanted to ask few queries as follows:
1) Her breasts are increasing gradually and dripping bit so whether she should discontinue wearing the bra? During her first pregnancy she weared bra for long time and so she face problems with milk from nipples. Milk had to be pumped out using breast pump which was very pain full for her.
2) Should she wear maternity bra or nursing bra now or better stop wearing bra itself?
3) We both have the urge to have sex currently. I like to kiss and suck her nipples and handle her breasts. So will touching or squeezing her breasts do some damage to them?
4) How frequently can we have sex during second trimester?
5) She had a small spot of blood 15 days back and after that doctor advised her bed rest.and limited travelling.Thereafter after god's grace nothing happened. We have again resumed sex now.

Hi XXXXXXX I have gone though your question and understand your concerns.I would answer your queries as you asked:
1.Breasts do increase in size with increasing gestational age in pregnancy due to the hormonal effects. The milk production also starts. She should continue wearing bra as it supports the enlarged breasts, and avoid pain. Moreover, wearing bra also prevents milk from flowing out. She seems to have excessive production of milk in breasts.This cannot be stopped as it is due to hormonal changes in pregnancy.
2. She should normal well fitting bra.
3. Handling and sucking breasts can lead to increase prolactin hormone production in the body, which increases milk secretion and will cause further discomfort to your wife. However, you can have intercourse.
4. You can have intercourse as and when you both wish.
5. Spotting in pregnancy can be due to low lying placenta, or local lesion in vagina and cervix. I would suggest her to get a ultrasound and get herself examined by an OBGYN. If these two are ruled out, you can safely have intercourse.

There is no low lying placenta or other issues as we checked 15 days before on the same day when the spotting occured.

Can her breasts increase in size due to handling or kissing?Because she doesnt like huge breasts and fears that due my handling they will increase more in size.

Secondly i and she like her nipples to be kissed during sex.Will it cause any harm or trigger labour at this stage?

Can we have sex every 4 days?Can we have it in third trimester?Because i heared in third trimester its not allowed.

Hi dear, thanks for responding. Breast handling and sucking the breast in pregnancy leads to increase in prolactin levels which increase the milk secretion. Milk secretion can lead to breast engorgement and enlargement.
Excessive sucking of breasts can lead to trigger of labour, however slight handling and kissing is not harmful.
Yes, you can have intercourse every 4 th day.
You can have intercourse in third trimester , provided it should not be vigorous.

I am not sure whether my followup question was sucessfully submitted or not. So here it is:

1) My wife had a query about her increased waist and breast size. She wants to know whether both these increased parts will return to normal after pregnancy or these changes are permanant. Her breast size was very small prior to first pregnancy, which started increasing gradually after first pregnancy and now its increased even more in the second pregnancy course.

2) Her weight in April was 58 kgs before the news of second pregnancy and now its increased to 62 kgs in the current month. Is this weight increase normal and will she grow fat at the ninth month if this goes on like this way?

3) Her stomach size was very small in the 4th month of the last pregnancy,but this time we see it much increased in the same month.She fears how much will be the stomach size in the ninth if this goes on like this way.

4) Is sex without condom safe during this period. We did it like this way only last week.

5) I saw on net that wearing wired bra is not advisable in pregnancy.So should she wear maternity bra or nursing bra?

Hi XXXXXXX I have gone through your questions and the answers are as follows:
1. The breast and waist size gradually increase in pregnancy and this comes to normal size, if at all, after at least2 years of child birth. Breast size gradually increase with each subsequent pregnancy.
2. Weight gain in pregnancy is approximately1.5-2 kg a month, and her weight gain till now is completely normal.
3. Abdominal size in pregnancy depends upon uterine size. Uterine size more than gestation age can be due to increased fluid around the fetus or due to big size of the fetus, these need to be confirmed on ultrasound. Moreover, the increase in abdominal size is more apparent on subsequent pregnancy as the abdomen becomes lax due to stretching in first pregnancy.
4. Yes, you can have intercourse without condom and it is completely safe.
5.She should wear normal well fitting maternity bra. Wired bra should better be avoided.

Thanks for the reply.

I told her all the answers and she is satisfied.

One thing I wanted to know is that as I said we intend to come close every 4th day.My wife is very sensitive to such thoughts and my touch.

So can this be the reason of the one drop of spotting which occured 20 days back?Although we did have sex after that and it was safe by god's grace.

The general norm followed by the doctor is to come close after every 15 days. But it will be difficult for us to follow such long gaps as second trimester is the only period when we can come close.

You both can have sexual intercourse every fourth day, there is no such period or interval recommended.
Sexual intercourse can be a reason for spotting if there was a low lying placenta or there is some local lesions in cervix or vagina. Since these are ruled out in your wife's case, sexual intercourse is unlikely to be the cause of spotting.
You can have sexual intercourse safely till 36 weeks of pregnancy.

I wanted to ask whether it is advisable to to intercourse after the insertion of miprogen or before?

My wife has been prescribed miprogen since last one month and she inserts it through vagina morning and night.

Does it have any effect on pregnancy if my semen with mix with miprogen?

Hi XXXXXXX it is preferable to have intercourse after the insertion of miprogen, and there are no adverse effects of the miprogen gets mixed with semen. Miprogen is progesterone which gets absorbed through the vaginal walls, however semen will not get absorbed through vaginal wall and entry into cervix and vagina is restricted by the mucus plug at the entry to cervix.

Hello,

Today we just walked into to a lingerie shop and the lady suggested a bra for pregnancy. I am not sure whether its a different bra or the one used normally.

But as I mentioned in my previous posts, during the last pregnancy, my wife wore normal bra till 7th month and the result was that her nipples didn't protrude as wanted and the baby couldn't latch up to them and discontinued the feeding.My wife had then required to use breast pump and draw milk which was extremely painful.

What are the steps we can ensure that her nipples can still function as required after pregnancy for the baby to feed normally?

2) As you said in earlier posts that the spotting may be due to vaginal or cervical lesion. But can these lesions be visible in Sonography because the doctor didnt reveal any such kind after the sonography.

3) If spotting has to be because if intercourse, does it happen immediately after intercourse? Its because the day this happened was Monday morning and we had intercourse on Saturday afternoon.

4) Is white discharge common in these days?

Hi dear,
I will answer your questions as you asked.
1. I suggest your wife to consult her doctor and get her breasts examined for nipple retraction, if they are retracted, then the baby will have difficulty in latching upon. There are certain measures which help in making the nipples prominent, and this have to be done prior to child birth, to prevent latching problems. Wearing bra do not effect the size or prominence of nipples.
2. Vaginal or cervical lesions are not seen in ultrasound. The doctor has to examine it vaginally.
3.Usually the spotting is immediately after intercourse. In your case, the spotting was quite delayed, then it is unlikely related to intercourse.
4. White discharge is increased in pregnancy, due to the effect of progesterone hormone which is increased in pregnancy.

The problem last time was her nipples were protruding out but just for few minutes and due to this the baby could not latch on to it.

Just for the knowledge what are the nipple retraction measures.Are they painfull?

When I kiss my wife nipples they protrude out.So if i do this at regular intervals would that help in nipple stimulation and retraction?

Kissing and handling nipples are not very effective methods for this purpose, though can help a bit.

Hello XXXXXXX Mam,

Today we had gone for Anomaly scan and I have attached the report with this email. The radiologist said there is a issue of mild oligohydramnios and low lying anterior placenta.

I am worried on account of both of these problems though the radiologist said report it normal.

How can the amniotic fluid level be increased and how about low lying placenta?How much we should be worried?

Hi XXXXXXX I have reviewed the ultrasound report.
The report mentions mild oligohydramnios, but it does not mentions the amniotic fluid levels which are measured in terms of AFI which is amniotic fluid index. This value gives a fair idea about the severity of oligohydramnios. However, she should take adequate rest, drink plenty of fluids( 3-4 litres a day) and have high protein diet. These measures usually increase fluid around the fetus.
Placenta is low lying, in the lower uterine segment,and this can cause bleeding in pregnancy. She should avoid heavy work, avoid lifting heavy objects and in this situation, intercourse should also be avoided as it may disturb the low lying placenta and cause bleeding.

1. Fluid around the fetus is low either due to decreased blood supply to the fetus or decreased Fluid intake by the mother, provided that fetal renal system is normal, which is shown to be normal in the ultrasound in your case.
To assess fetal blood supply, your wife should get a Doppler ultrasound done.
Decreased fluid around the fetus causes decreased growth of the fetus.
2.Intercourse should be completely avoided till theuultrasound after4-6 weeks show upward migration of placenta.
3.She can walk normally, but running and jogging should be avoided. Traveling should be generally avoided until very necessary.
4.Yes, oligohydramnios and low lying placenta makes her pregnancy to be a high risk.

If no intercourse then whether just kissing her lips or kissing her nipples frequently will have any risk?

Secondly she generally has a constant urge for urination. Is this linked to this situation?

Constant urge for urination is common in early pregnancy due the pressure of a the uterus on the bladder. Now, it can be due to urinary tract infection. I suggest you to get her urine routine and microscopic, and urine culture examination done to confirm the diagnosis and treatment should be done accordingly.
