Suggest Treatment For Prostatitis
Question: Dear sir, my father is already a heart patient and in a recent routine blood test, the report indicated the PSA is 14.55. So we took ultra sonic scan, these r the details
Post void residual urine 39cc
Prostate measured 4.1 x 3.6 x 3.3, vol - 25.8
In the report in bold letters they said the following
Prostate is mildly enlarged in size and shows normal parenchymal echo texture.
Impression;
Mild prostatomegaly with no significant post void residue.
The uroligist suggested uroflow test, then after 2 weeks of tablets again a PSA test and if required a biopsy.
Kindly let me know is it any thing alarming of a cancer? He is 80 years and iam not sure of how to deal with this as he is a heart patient

Thanks for query.
Your father has raised Serum PSA (14.55).There is a general understanding that raised PSA is suspected to be due to cancer of the prostate which has to be ruled out or confirmed by doing ultrasound guided Prostate biopsy.
However one gets raised PSA due to infection of the prostate (Prostatitis).Hence I would suggest you to get his urine culture done and give him appropriate antibiotic like Cefixime along with urinary antiseptic like Nitrofurantoin for 2 weeks and repeat his serum PSA .
If it turns out to be still high then you will have to go for Biopsy of the prostate
Looking at his age and heart problem you need to consult qualified Uro -Oncologist for medical management of cancer of prostate depending upon the report of biopsy of Prostate.

Dear Dr XXXXXXX thanks for your quick and detailed answer.
Yes the Urologist gave 2 medicines for next 2 weeks and again to re-do the PSA test , and based on that he will take call for the biopsy, he said after checking the uroflow test this morning that its ok not alarming but still to be on the safer side to do the above procedure of medication.
