Suggest Treatment For Neurotic Health Anxiety With Depressive Proness

Question: Hi Doctor,

I was recently diagonised with 'Neurotic health anxiety with depressive proness. No impression of any suggestive Psycosis' by a registered Psycologist. He took 2 tests as recommended by a Psychiatrist, and confirmed that I don't have psychosis. He mentioned I only have a transient Medical Student Disease or Nosophobia.

Since the doctor had recommended me 0.5 mg risdone previously, I thought whether the medication was administered for any Psycosis like Scizophernia. Later, as confirmed by the psycologist and psychiatrist, I don't have any psychosis and only have neurotic health anxiety with depressive porness.

Now I have the below doubt.
In people with Schizophernia, it is said that, they hear voices. Of course I don't hear voices.

But,how can one differentiate between thoughts and hearing voice?
Say for example(nothing like this ever happened as mentioned in this example), you are in a classroom and the class teacher is new person. But the class teacher gets acquainted with names of all the students even before he enters the class for the first time from the previous class teacher. When the class teacher points you and calls you by name, you might be taken by XXXXXXX and would have a thought 'How come this person ever know my name, even before I got introduced to him' . This is just a normal thought right.

I called the psycologist and he told, we all initially feel like this. It is just a normal thought and not any symptom of hearing voices. He also told hearing voices will be Auditory, more external to the ears. Also he told sometimes bathroom singing and humming a song is also normal and everybody does that sometimes. In some websites it is written bathroom singing and humming is not normal. Is it true? I know many of my friends who sing and humm a favourite song in bath room sometimes.

Kindly provide your insights on my queries.

Thanks & Best Regards,
XXXX

Brief Answer:
Hi, Not worry.

Detailed Answer:
Hello,
Thanks for your query.

It does not appear auditory hallucination to me. Bathroom singing and humming is normal. Do not believe in any websites. Better to discus with your psychologist and psychiatrist, which you usually do.

Auditory hallucination is frightening experience for patients. They used to become fearful and search source of voices, as voices appear real to them. They do not insight regarding problem or voices.

I hope this information has been both informative and helpful for you. In case of any doubt, I will be available for follow ups.
Wish you good health.

Thank you Doctor, the information was helpful. I had already checked this with my Psycologist. He has also told the same thing to me that it is our own thoughts, like I thought of writing the query to you. He told thoughts are different and voices are different. Since I am not feeling like I am hearing any voice or somebody talking to me or any unusal sound, can I confirm that I don't have any Auditory hallucinations? The psycologist has already confirmed the same but I need your word.

He only accounts these doubts from me because of anxiety issue which I have and not any auditory hallucinations.

Hi Doctor,

As discussed during our previous discussions, you know that I have 'Neurotic Health Anxiety with depression. No impression of any suggestive psychosis'. This was evaluated by a registered psychologist.

Now I have the below query to you. It is written that disorganized speech is one symptom of schizophrenia. What is a disorganized speech? Please give me an example.

From our previous discussions, can you evaluate and confirm to me that, I don't have any disorganized speech. Psychologist and my consultant psychiatrist say that it is only my anxiety and my speech is not disorganized. But I require your confirmation that, my speech is not disorganized.

Thanks and Best Regards,

Brief Answer:
Hi

Detailed Answer:
Hello,

In schizophrenia disorganized speech means that patient have inability to express his thoughts properly. Commonly following problem is seen in speech:
-Neologism: use of meaningless new words for some meaning.
-Circumstantiality: Use of excessive useless description to express simple thought.
-Tengentiality: Giving excessive information regarding things related to topic but not about the topic itself.

These disorders also become manifest in matter written by patient. In your all communication I have noticed it. So, do not worry about disorganized speech in your case. In neurotic disorder some stammering and hesitation in speech is common finding.

Take care.

Hi Doctor,

Since you have found some disorganized speech in my writing, do I need to go for evaluation of Schizophrenia again or can I just console myself thinking it is only a side effect of having Neurotic health anxiety?

Kindly suggest.
