General & Family Physician 's  Response
Hello,
I have gone through your query and understand your concern.
In Ultrasound, the cortex (outer portion of the kidney) and medulla (inner portion) can be differentiated due to specific cellular and tissue structures.

But due to any long standing medical conditions which caused chronic kidney disease, can result in tissue changes, with reduced renal function, and this results in appearance of both the cortex and medulla as similar zones and cannot be separately identified.
This is called decreased Cortico-medullary differentiation.

Treatment in such a condition, would be to treat or control the underlying cause like Hypertension, diabetes etc.. for the chronic kidney disease.
Take all the medications prescribed by your treating nephrologist,
Reduce your sodium intake.
Exercise regularly.
Do not smoke or use tobacco.
Do not drink alcohol.

Hope this helps.
Please write back for further queries.
Wishing you good health.
