My doctor has sent me for immediate scan, I have a heart failure condition, I reported today with swollen ankle which I have had for some weeks, quite painful , he states on the phone my dimer count is quite high,? what does that mean? and if it is a blood clot how can it be dissolved, I stopped taking warfarin over a year ago and take quantities of garlic!! what immediate medication would be required if it is a blood clot?

Hello. Thank you for your question and welcome to HCM. I understand your concern.

In case you have been on warfarin, there was a clear reason why you needed "blood thinners". They are used mainly when there is a clot in the body or when there are heart conditions that predispose clot formation. In both cases there is the same aim - preventing a clot particle to go and block small calibre peripheral arteries and damages the areas supplied by them. This is why oral anticoagulants are used. If you quit using them, then the predisposition for upper mentioned phenomena is increased. A high D-dimer is indicative of some thrombotic event in the human body, but combining with particular symptoms, we usually recommend scans of a particular area, to confirm or exclude areas that clot particles could damage. The treatment in these cases are unfractioned heparin or thrombolytics, such as streptokinaze, reteplase, alteplase, tenecteplase etc. Of course there are certain counterindications to these drugs, which will be cleared by your attending doctor.

I hope I was helpful with my answer. Kind regards.

Dr. Meriton
