Hi,
After going through your brother's report there is Pyelonephrits (infection of kidneys).
I want to know
1) His age.
2) Any associated illness like diabetes.
3) Has he any problem while passing urine.
4) USG KUB report prior to start antibiotics if done.

I will advise -
1) Hospitalise him.
2) He may require Intravenous antibiotics depending on culture and sensitivity report for longer duration.
3) Blood test like complete blood count, kidney function test, urine examination including urine culture and sensitivity and other test as required.
4) He require close watch as infection may land him in sepsis.
