General Surgeon 's  Response
Hi,

Noted your history about chest pain and shortness of breath for which you went to ER and they found out pericardial effusion, advised drainage of fluid, symptoms remain the same in spite of D dimers. CT scan has shown no clot means no embolism, possible pericarditis might have been a sequel of pericardial effusion. 12 mm spot on CT scan must have been reported as possible cause and your Doctors must have thought of some disease that caused such a spot. This can be due to small lung abscess, past embolism causing fibrosis; tuberculosis should have been thought of as well as other granulomatous lesion.

You need the following in such a situation:
Get an opinion of a Pulmonologist.
Review limited scan for the lesion.
Guided FNAC if possible.
Bronchoscopy and lavage to be sent for lab tests to know the possible cause.
Relevant tests of blood.

All these will lead to a proper diagnosis and thus allow for proper management.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
