Hello and welcome to HCM,

At the age of 46 heavy bleeding can be due to hyperplasia with or without atypia and cancer of the endometrium.
The ultrasound of your pelvis reveals thickened endometrium i.e. 7mm.
The thickened endometrium can be the cause of heavy bleeding.
The endometrial glands can show atypia and atypia is a harbinger of cancer.
Thus, removal of uterus will remove any abnormal endometrial lining which can give rise to malignancy in future.

The uterus removal surgery can follow the gall bladder removal.
Proper intraoperative and post operative care will prevent complications associated with surgery.

Thanks and take care
