What Does An Involutional Change And Small Vessel Ischemia On CT Mean?

What does a moderate involutional change and small vessel ischemia on CT signify with regard to considering a diagnosis of vascular dementia in a patient with insidious progressive short tem deficits over a year which is unrelated to falls /HTN or stroke disease?

1.Small vessel ischemia is seen as a age related changes, with underlying medical conditions like diabetes, hypertension, vascular pathologies.
2.Smoking can be one of the possible cause, atherosclerotic changes and so on.
3.In this case first thing to manage and maintain the underlying pathologies, which gradually improve the status.
4.Progressive dementia with gradual progressive deficits are seen in case of the narrowing of the vessels.
5.I would recommend to get a carotid doppler to see if any atherosclerotic changes / plaques and treat accordingly.
