Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

Where is the clot in the urethra or penis or on the prepuse. It may pearly penile pampule or engorged vein or injury to the vein which may aggravated on erection or it may be due to other causes. Until examination is done it is difficult to say what it is possible red spot or herpes infection or erythema etc. Please consult your doctor he will examine and treat you accordingly.

Hope I have answered your query. Let me know if I can assist you further.
