What Causes Swelling And Hardness In Left Side Abdomen?

upper left abdomen is swollen, its hard. constant dull pain. sometimes a pain will be sharp up near my shoulder, chest, or back. it started when i had broncitus. due to caughing i had a herniated lung. they say its rare how mine was. it did not go into the diaphram instead it went between two ribs. ive had surgery to fix that but the pain on my left side is still there and it was not swelled before the surgery. i have had the ct scan and the ultrasound and the came back fine.i have no problem with my bowels. the doctor does not know what this is. do you have any suggestions?
