General & Family Physician 's  Response
Hello,

You will need a good physical examination to determine if the sensation is due to muscle spasms or due to a pinched nerve. Your doctor will need to feel your muscles at the back and also will need to do a neurological exam to have a better understanding of this.

In both cases, physiotherapy may help to relieve the sensation as it may help the muscles relax and also decompress the pinched nerve.

I hope this answers your query.
I remain at your disposal for further medical assistance.
