What Causes Pain In Left Buttock And Leg?

I have a lipoma near my spine and in need of lumbar steroid injecion for an annular tear and buldgind disk L3, L4. I have had successful core decompression in both hips for Avascular Necrosis right hip in 2002 and left hip in 2003. I had a partial laminectomy L4,L5 in 2001. I had a steroid injection in 2006, two injections in 2009, and bilateral steroid injection low back this past April 2011 which helped my right side, but pain continues in my left buttock and left leg. My doctor recommends a second injection. I also have occasional pain in the left groin area and left hip area. Should I be concerned about recurring AVN or concerns about the lipoma. My doctor commented that I had a lipoma at my follow up appt. after this last injection. My quality of life is being affected by this pain.
