General & Family Physician 's  Response
Hi,

Smoking cigarette may cause chest pain due to blockage in the arteries known as angina or damage to yoùr heart muscle because of lack of blood flow to your heart known as MI or any respiratory diseases like pleurisy, pneumonia etc.

Again, smoking cigarette may lead to develop Buerger's disease which is a form of vasculitis that inflames the blood vessels of hand and causes both tissue damage and pain. Treatment depends on causes. You should undergo some investigations like CBC, CRP, ECHO, Duplex study of upper extremity, CXR P/A view, RA test etc to find out the underlying causes for your present sufferings and then treat them accordingly.

Take care. Hope I have answered your question. Let me know if I can assist you further.
