I am Dr.Saddiq ul Abidin. I have read you question completely, i understand your concern and will try to help you in best way possible.
The kind of symptoms you have shared, are suggestive of right flank or lumbar pains, which raises the possibility of a
renal colic on the top among the lists of differential diagnosis. Although the associated history of burning micturation, frank blood or puss in urine or fever may or may not coexist.
The comorbidities like
diabetes and
hypertension, also raises the possibility of a
kidney pathology. I would suggest you to start the work up with a urinanalysis, along with kidney function tests preferably with a Complete blood profile too, if possible. Though you have mentioned, you don't have a previous history of kidney disease, but such symptoms can come with some acute change of events as nothing can be ruled out at such premature stage. Also that, the need of doing routine Kidney tests along with urine analysis is mandatory for diabetics, every three monthly.
If symptoms persist, you can have an
ultrasound or X-ray KUB, to rule out kidney stones as well as any surrounding sub-diagphragmatic
abscess. Even if urine analysis doesn't show blood or ultrasound and X-ray KUB is not suggestive, still small concretions which are left out of these conventional detection methods, can still be present, which normally flush out with proper hydration and excessive water intake. For severe discomfort, you can use an antispasmodic, like Nospa or Hyoscine after your doctors advice.
Liver capsule or any cyst or lesion in
right lobe of liver, can also sometimes present like this, but that is a rare possibility. That can be ruled out, on an ultrasound abdomen and Liver function tests.
The high likelihood of a muscle spasms, without any history of trauma, can be possible, for which you would need to be examined in person with detail by your doctor. In such case, rest, muscle relaxants and pain killers would suffice your symptomatic relief and spontaneous recovery.
