What Causes Itchy Bottom Along With Blood In Stools?

Hi my son has had annitchy bottom for about 3 weeks now. We have treated him with worming tablets and still no change. He is generally waking during the night in pain with the itch. I have also noticed in the last 5 days there has been blood in his stools and when I wipe him. What could be the possible cause? I have called a locus and he had no idea. Tina
