Hi,

Scratches in your penis can be due to
1.     Infection - it can be fungal infection considering your complaints if you find dry skin flakes mostly it's fungal if you have discharge we need to think of bacterial or viral usually shrinking of the penis is very rare unless the infection decreases the elasticity of penis skin
2.     Dryness – due to infection can cause injury and scratches, improper foreplay causes dry sex leads to pain and inflammation, dehydration due to inadequate water intake, age factor, hormonal imbalance
3.     Forceful sex can cause injury and inflammation
4.     Use of sex toys or a particular brand of the condom can be allergic 5.     Vaginismus - the constriction of the vagina during sex leads to painful sex and injure the penis
6.     Eczema

Delayed erection or Erectile dysfunction since its only a 3-week history disease-causing ED can be far stretched. Few reasons for your ED can be due to
1.     Decrease sensation in your penis
2.     Recently u had masturbated
3.     Stress
4.     Depression
5.     Fear of failure
6.     Low self-esteem (Chronic Masturbation)
7.     Homosexuality
8.     Religious constraints
9.     Long term illness
10.   Negative attitude towards your partner
11.    Hormonal issue
12.     Long term drug usage.

Hope I have answered your query. Let me know if I can assist you further.
