A standard menstrual period is from 21 to 35 days.
Bleeding time lasts 5 days and amount of blood is about 40-80 mL.
Menorrhagia is unusual
uterine bleeding (AUB), the amount and duration of vaginal bleeding are abnormally high. There are many causes for painful menorrhagia including infection,
endometriosis (endometrial material exterior), adenomyosis (endometrial tissues move inside the uterine wall), abnormal IUD placement, etc.
To rule out the reasons you may need Pap smear, pelvic ultrasound scan,
endometrial biopsy, urine analysis and culture, routine tests, thyroid tests, etc. For this purpose, visit your gynecologist.
To me, regarding your symptom presentation, I think you may need to remove your IUD if you face to following problems: persistent irregular bleeding, spotting and pelvic pain, abnormality in IUD placement, and also enormous, recurrent and drug resistant infections.
Additionally, given that some abnormalities found in tests mentioned above may lead you to IUD removal.
