Hello,

The issue of intense pain, itching in vagina and heavy menses sound to be related to vaginal or cervical infection with some sort of bacteria or fungi most likely. This is also quite frequent even in those who are not engaged in sex activities as well. My guidelines at present looking towards the severity of the condition:
- Drink more liquids as you can
- Take cranberry sachet or tablet twice a day
- Raise intake of citrus fruits with vitamin C as natural source
- Take utmost care of vaginal hygiene

- Avoid prolong use of same tampons
- Avoid use of public urinals till possible
- Take care to wipe the vagina from front to back after each act of urination, as reverse is associated with very high chances of transmitting peri anal bacteria to vagina and urethra
- Control of blood sugar

- Prescription of anti histamine as cetirizine, antibiotics and anti-fungal medications as per the consultant choice
- Most important to get a culture report of urine and vaginal swab tested in microbiology lab for assessment of type of organism responsible and further management in accordance to that only.
- Avoid masturbation (if active)
- Avoid smoking (if applicable)

Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
