What Causes Elevated ALT And AST Levels Along With Enlarged Spleen?

Hello I have become a pin cushion over the last year trying to figure what is wrong with me I hurt so bad everyday I have elevated ast, alt,esr,and my spleen is enlarged, i am gaining weight mainly in the stomach but its not fat its like bloat and hard i have ibs with constipation swelling in hands and feet brisk reflexes abnormal gate what disease could this be?

It may be due to liver pathology.
you may need clinical correlation,viral markers and USG of abdomen.
