General Surgeon 's  Response
Hi,

Exploding headache, particularly on standing or tilting the head, on coughing can be due to hypertension, sinusitis, intra-cranial blood vessel malformation, tumors, or cervical spine problem with blood vessel problems. Hence, the need is proper clinical evaluation, examination of ENT, neurology. Investigations as per the suspected cause to get a proper diagnosis and thus have a specific management.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
