What Causes Cough And Sudden Weight Loss While On Xarelto?

My husband is 79 years old. He is completing cardiac testing. He has had a chemical stress test, ECG, Holter monitor, EKG, and blood work. We are waiting for results. Last year he was diagnosed with multiple pulmonary emboli and DVT in left leg. He was put on Xarelto and continues to take those. He is now also taking diuretics to reduce water retention because he has gained almost 20 lbs in 4 months. He has lost 4 lbs in 2 weeks. His lower legs are swollen and pink. He is tired almost all the time and frequently falls asleep when he sits down. He has a hissing in the ears. Yesterday he developed a cough ( dry, nonproductive). Today he is still coughing and has had a few dizzy spells when getting up from the chair. His follow up appointment with the cardiologist is in 3 weeks. Should we wait or is this something that is more urgent and needs to be looked at earlier?

Internal Medicine Specialist 's  Response
Hi,

I would suggest you visit the ER and have his oxygen saturation and arterial blood gases checked if he is getting enough oxygen.

Although it is unlikely to develop pulmonary embolism while being treated with Xarelto.

I would also like to get a chest X-ray done to look for any fluids in the lung or pulmonary oedema and other diseases like pneumonia.

His blood pressure should also be checked both in lying and standing position (to look for postural drop) to find we aren't dehydrating him by giving extra diuretics and hence having dizziness.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
