Welcome to HEALTHCARE MAGIC..

So sorry to hear about your problem and will try my level best to resolve it to the best of my knowledge and experience..

As per your complain it seems that whitish appearance over tongue with red spots and along with it you do not have the habit to brush your teeth often that is indicating that you do not have a good maintenance of oral hygiene..

The whitish appearance of tongue can be due to causes like accumulation of debris over the tongue and more of bacterial and fungal growth..
If the layer is in the form of a thick cheesy coating it can be due to yeast infection known as Oral Thrush.

Red spots can be due to swollen tongue papillae while can also be due to formation of canker sores and as you traumatized them with toothbursh there has been bleeding and now there is sore formation as well as inflammation..

I would suggest you to consult an oral physician and get evaluated and a thorough clinical evaluation and investigations like microscopy of the scraping from tongue can be done for diagnosis and treatment can be done accordingly..

In case if it is due to debris accumulation, then gargling with 1:1 solution of hydrogen peroxide and water can help..
You can also use a tongue scraper to clean your tongue daily once the sores resolve..
Chew more of hard foods as they clean the tongue with frictional action..

In case of thrush you will be advised oral antifungal medicines like Fluconazole along with Nystatin mouthwash, Clotrimazole lozenges and plenty of probiotic intake like Yoghurt..

Hope this helps..

Regards..
