General Surgeon 's  Response
Hi.
The pain in the lower abdomen which start 2 days before the menstrual cycle and persists for one week can be due to the congestion causing increases pain in the Pelvic Inflammatory disease or may be endomteriosis.
The ultrasonography is suggestive of something like this.
A therapeutic trial with a course of an antibiotic, metronidazole and anti-fungal may help.
I would advise that your spouse / sex partner to take the same course , dose and duration.
No sex for 10 days minimum.
IF this helps the diagnosis and treatment both become significant.
