Suggest Treatment For Weight Loss And Lack Of Appetite

I was diagnosed in May 2016 with rhuematoid arthritis, my ANA level was very abnormal and they are having me to be checked by a arthritis specialist. My concern is they think I may have lupus. I was wondering what will be expected with this disease and what will be in my future. I work as a RN and my joints are often very achy and stiff. I am only 52 years old and shuffle with feet on my days off. Limited range of motion in bilateral shoulder joints, knees, hands, fingers, and especially bilateral hip pain. Sometimes when I am walking my hips feel as if they are going to pop out of socket. This has really hit pretty hard in the last year and the stiffness and pain is sometimes excruciating. Some days are not as bad. I knew my joints hurt and that is when my primary decided to test my ANA and it was positive for rhuematoid. However, I wonder since they are thinking Lupus, what is in store for me? Such as treatments, exacerbations, etc...Do have a appointment in October, the first available they had. Just wondering about my weight loss and hot flashes: Menopause and lack of appetite sometimes??

General Surgeon 's  Response
Hi,

Based on your symptoms, I would like to rule out anemia and thyroid hormone imbalances. Gastritis could also cause this (but has been ruled out by endoscopy).

I recommend: (please exclude those already done)

-blood tests (complete blood count, peripheral smear, TSH, free T3, free T4, Vitamin D levels, kidney and liver function tests, lipid profile, blood glucose levels)
-stool analysis to check for parasites and occult blood
-urine analysis for protein loss

Based on the above tests and clinical exam findings, I could help you further.

May I suggest:
- try to eat bland and small meals every 2-3 hours. Eat even if not hungry.
- avoid caffeine/carbonated and sugary drinks
-avoid spicy, oily and fatty food till you feel better.
-if nausea is preventing you from eating, keep something like lemon, orange or mint next to you to curb nausea if you feel it. (poke a few holes in the fruit and smell its essence)
-Have cool milk and sips of water throughout the day. Stay well hydrated (urine should be pale color during the day).

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Ivan R. Rommstein, General Surgeon
