Diabetologist 's  Response
Dear sir i can understand your concern
in my opinion the surgeons view is very correct and to control diabetes you need to start insulin and slowly increase the dose until sugars come down.
blood supply and sensitivity of nerve should be tested for knowing prognosis.
the test available are
1.vibration perception testing
2.ankle brachial index
its very important to check for vascularity as good vascularity will make wound to heal faster and better.
giving oxygen at site of wound is a new kind of treatment which will help in quick healing of wound.
its mandatory to be on good antibiotics and debridement as per surgeon
