Hello,
Thank you for asking at HCM.

If your big toes are tender and swollen and your doctor prescribed indometacine and allopurinol than the mist likely diagnosis is gout.
Gout can recurr and there are some things to do to prevent it:
1- totally avoid alcohol
2- reduce protein rich foods like meat, eggs, diary products, beans, etc
3- do not overuse your legs and feet- this is the time for some rest from exercises if you have been exercising
4- take your medications regularly as prescribed
5- some patients might need allopurinol and low dose indometacine for a few months or another option is allopurinol and cholcicine for a few months. if you try to stop early than recurrences happen.
6- If you suspect of other diagnosis than x-ray and blood tests might be helpful to rule them out (complete blood count, ERS, RF, ANA, tests for rheumatoid disease)
7- meanwhile you can help with pain with RICE measures: R- rest, I- ice, C- kill pain, E- elevate the legs when sited as much as you can.

Hope this is helpful and answers to your question

Best regards,

Dr Antoneta
