Suggest Treatment For Recurrent Nummular Eczema

hi, im dhen 23. i got a ringworm in my left arm for a year,.my first 3 differnt doctors and each of them has a same diagnosed, they diagnosed it as nummular eczema, so each of them at a time give me a tropcl cream, steriods, and even prescrbe med such as loratadine plus prednisone,for 2 weks it clears up.. after a week or so it pops up, in addition she change my meds into prednisone plus montilukast and again after two weks it fades.but luckly afyer a wek it pops again.. and so my last doctor refer me to a dermatologst, my current dr diagnose it as ringworm. so she prescribe me an oral med namely Lamisil 250 mg for 2weks and calamine lotion to be applied in an effected area. yesterday was my last dau of lamisil and wen i meet my dr yestrday she said, iys ok ur done taking lamisil so heres what u should do, clean the area,,then apply CANDIVA cream and after it was absorb by your skin apply desonide cream for redness ,and no need to wash it off, just applied it then again when your skin already absorb the chem of candiva plus desonide apply it with calamine lotion for itchness abd skin that peela off, so it appears that as if it was a layer or series of 2 ointments and 1 lotion. now Dr. help me please, i want to know if these is really effective in treating my ringworm..mplease please help me...god bless and kore power

Hi,

The treatment of ringworm given to you is fine however in my opinion the Tab Lamisil (terbinafine) which is an anti-fungal given to you was for a short duration however if the lesion has vanished then its fine. If the lesion is still there and you feel it hasn't improved much then you can continue the Tablet for a longer period, up to a month or 2. Now you are applying the combination of an antifungal ointment Candiva (clotrimazole) along with Desonide which is a steroid and it helps in reducing the inflammation (swelling, redness etc.). Calamine has been given for reduction of itching and also for providing little soothing feeling over the area. All these medicines are fine.

I would advise you not to apply the steroid ointment (Desonide) on a regular basis as it causes the skin contour to go and skin becomes more scaly, if applied for a longer duration. You should apply it only during flareup of inflammation of that area. Please review with your treating doctor with my opinion and continue only with Candiva for a long time, up to 3 months, so as to avoid recurrence. In addition to this, I would advise you to get a sugar fasting and PP (after breakfast) test done to rule out diabetes as fungal infections have a predisposition to affect individuals with diabetes more.
