Suggest Treatment For Pain In Lower Back And Legs

Hi, I was diagnosed with nerve impingement and am scheduled to see an orthopedic surgeon this week. I take 2 Tramadol 50mg every 4hrs or as needed for pain no more than 8 pills per day. Not able to sleep at night due to pain in lower back and down my leg to my toes. Usually wake up after 4hrs of sleep and not able to get back to sleep.Do you have any recommendations?

General & Family Physician 's  Response
Hello.
Pain in the lower back pain may sometimes be associated with compression of the sciatic nerve which may result in severe pain radiating to the back of thigh, legs up to the toes. other signs like tingling and numbness may be noticed. An MRI of the lumbar spine would be needed to exclude the chances of lumbar spondylitis and disc prolapse/bulge. OTC Pain medications like acetaminophen or ibuprofen OR prescription strength like tramadol, diclofenac may be needed keeping the age and other parameters in mind.
Physiotherapy along with back muscle strengthenimg exercises, epidural steroid injection in the back would be helpful. MRI lumbar spine would help take the next step. Good to know that you plan to visit the orthopedic surgeon. Please avoid sudden changes in posture, bending, sleeping on a soft mattress, lifting heavy weights, etc. It would be best to sleep on a hard mattress in a straight posture. Hope this helps and you feel better soon!
