General & Family Physician 's  Response
Hello,

I would advise you to have an ultrasound examination of your testicles as you have swelling in the area with bowel movements. This will help rule out a hernia.
Also, taking urethral secretions for chlamydia and gonorrhea is advisable to rule out sexually transmitted infections.

I hope this answers your query. I remain at your disposal in case further medical assistance is needed.

Regards,

Dr. Antoneta Zotaj
