Suggest Treatment For Endometriosis, Lower Abdominal Pain And Fatigue

Hi, I am aged 26 and have suffered with endometriosis for around 13 years. I am sick every month for 1-2 days, keeping water down is hard, i can not eat for the first day mostly. I am aware i am sick due to the pain, if that is correct? The pain is in my lower abdominal area especially. Acute, dull and throbbing. The pain is also in my lower back, thighs and bottom. Each month the first and second day of my period reduce me to shouting or screaming out in pain, i have to take a very hot bath, sometimes 6 times over 24 hours as it helps relieve the pain whilst IN the bath. I am fatigued, i can not stand straight at the time, as standing hurts my tummy. I can not stand the light at those times. I had key hole surgery when 15. The doctor told me they had found An old spot of endometriosis but did not burn away the spot, was that due to my age at the time and it not being what they considered as being severe? Since then the pain is just as bad if not worse. It now affects me for around 4-5 days of the month, sometimes a week to fully recover, every single month. I feel my life is on hold, sometimes. I have tried the combined oral pill, which resulted in me clotting and bleeding for over two weeks with pain that increased daily until i had stopped taking them. Mefinamic acid, pain relief, tranexamic acid, and anti sickness tablet (under top lip) to prevent sickness. Nothing seems to work for me up to now. Would you please suggest something? There is another issue , as endometriosis is a hormone condition, am i correct there? There is a likely hood of depression, low moods etc...due to the load of the condition, and the hormone reactions themselves? Most months, especially now i am older i have severe PMS each month. My moods are very irrational, high aggression, empty and void, no purpose. I can just cry and cry and have Irrational thoughts that are not logical and feel suicidal for that time. For example, At these times i may think someone is thinking a certain thing about me, always negative. I become convinced and feel i am 100 percent sure in what they are thinking, normally horrible things about me. In another example, this is more recent but not happened to me before, not like this. I was home alone and due on a cycle. I was convinced there was another person in the house with me. In my head there was a man in the house, at first i became scared as i thought Does he want to rob us and hurt me? I then completely changed in mind and thought assured myself he did not want to rob or hurt me but was homless and cold....it sounds absurd i know but it was how i felt. When my sibling came home i told them i what i was feeling/ thinking. A day or two later (maybe three) i come into a cycle and that is when i am aware of the reality of it. I know there was no one in the house now and look back in horror at my behaviour when i hit a period. I avoid going outside and sometimes can not get outside due to pain so a lot of it takes places in my home. Sometimes the neigbours hear me screaming in anger and internal pain, with me shouting at my family about wanting to hurt myself. That also takes me to swallow my pride fir when i am rationally thinking again, to go out.. Thank you for your time and reading my very long message! Have a wonderful day!!! :)

General Surgeon 's  Response
Hello,

The endometriosis is abnormal implantation of endometrial glands over serosal surfaces of ovary, fallopian tubes, adnexal regions, intestine , bladder or rectal surface. As there is cyclic hormone changes and shedding of the epithelia surfaces, episodic pain occurs involving lower abdomen area. Primary management with hormone therapy and in refractory cases, radical hysterectomy with debridement is mandatory in many cases.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
