In my opinion your drugs and dosage needs modification. Seroflo is having salmeterol and fluticasone. Salmeterol is long acting. Its action last for 10-12 hrs. Same with fluticasone. So serflo should be taken bd. And duolin is levosalbutamol and ipratropium. Both are short acting and action lasts for only 4-6 hrs. In my practice I prefer seroflo bd with tiotropium (very long acting, action last for 24 hrs) od and levolin on sos basis. And I always prefer MDI with spacer over simple MDI and DPI. Few more things that will help in your COPD.
1 avoid smoking if you are smoker.
2 get done adult vaccination like H. Influenza and Pneumococcal.
3 start pulmonary rehabitation in the form of chest physiotherspy, yoga etc.
