Hello and welcome to healthcaremagic.
I have read your question and I can understand your concerns regarding your brother.

What is his age..??
Has he got past history of tuberculosis or family history of tuberculosis.?
Do you have family history of inflammatory bowel disease such as chrons disease or ulcerative colitis.?
Any family history of carcinoma rectum..??

If i were his treating physician i would have done his digital rectal examination to look for any haemorrhoid, polyps or any hard mass arising from anal and rectal region.i would have done proctoscopy.
Then i would have done colonoscopy to look for polyps, any mass suggestive of carcinoma, ulcers, polyps, diverticular disease and inflammatory bowel disease.

I would have done carcinoembryonic antigen to screen carcinoma of colon.

For symptomatic treatment i would have suggested blood transfusion to replenish the blood going in stools which causes anaemia.
I would have prescribed tranexemic acid for controlling haemorrhage.
I would have prescribed iron and vitamin B12 suppliments.

I would suggest you to meet a gastroenterologist for workup and remedy for the symptoms

Thanks.
Hope the answer is helpful.
