Suggest Treatment For A Pilonidal Cyst In The Groin Area

Hello, I m Fadwa from Morocco and I would like to have your opinion about my case. It all started ten years ago when a pyogenic granuloma appeared in the middle of my pubis. I went to my regular dermatologist and we tried many rounds of antibiotics. It didn t work so I had a surgery to remove it. A few days after my surgery I had a little relapse but this time we treated it successfully with antibiotics. For the following eight years it was globally controlled... sometimes in summer when I would swim a lot the scar would get inflamed again... but I dealt with it by using an antibiotic cream and the inflammation always stopped. Sometimes I would even notice a few hairs coming out from the middle of the scar, but my dermatologist didn t seem worried by this. Two years ago I had again a little inflammation on the scar... It started a Thursday... I used the same cream as usual but this time it didn t work. It was like an abscess but it didn t have a head. By the weekend the swelling got so bad that it affected also my labia. All my external genital parts were swollen. I couldn t sit or walk without pain. I couldn t do anything at that moment so I waited until Monday morning to see my dermatologist. She told me she never saw something like that... she gave me a lot of medication and sent me immediately to a nurse in order to get shots of antibiotics. The mistake she made is that she didn t incise the abscess to release the pus. By Wednesday night the pus got out but unfortunately from the wrong place... it came out from the part up the clitoris, just under the top point of the labia majora. I went to an ob gyn who told me that my body created a fistula to release the pus, and he tried to open the center of the abscess with an unadapted instrument, it was so painful that I never got back again. I decided to go back to my surgeon who removed only the affected skin (a pyogenic granuloma appeared again on my old scar at this point) on the pubis. He didn t believe me when I told him I had a fistula so he didn t took care of it. He didn t even want to give me explanations about what he did during the surgery... I discovered I still had the fistula when I got another abscess at the same place less than two weeks later. So I decided to see another ob gyn this time. For the next five months we tried many rounds of antibiotics... everything failed. I saw him at least once a month. At the end he told me I had to try a fistulotomy without answering my questions about the process. So I got another surgery (the third one)... he opened both the extremities of the fistula and placed an elastic. Three days later the torture sessions started. The first time I thought I was going to pass away with pain. This whole fistulotomy thing was the worst experience of my life. After a month, the elastic was almost falling so he tightened it for a second to remove it. Unfortunately not even another month later I got an inflammation and a swelling at that exact place where he last tightened the elastic. I called the doctor immediately before letting the swelling worsen, it was before a holiday so I couldn t have an appointment. He told me to take an antibiotic and an anti-inflammatory drug. It got better with that treatment but I still got an inflamed pimple in the middle of the scar and scabbing on the rest of it. For the two next months I decided to forget about it, everyone around me was telling me that it didn t heal because I was focusing too much on it. So I just let it be. After those two months I noticed that the scar still wasn t healing and that the scabbing got worst. I went to another dermatologist this time (a professor) who was horrified and said she couldn t do anything for me but managed to call herself an ob gyn she knew in order to get me an appointment as soon as possible. That ob gyn gave me a treatment which worked well for a few months (Dermoval : dermo steroids). The scabbing disappeared and the inflamed pimple almost vanished. But less than five months later It started swelling again. Went to the ob gyn again, he drained the pus and gave me another topical treatment. It worked again for a month, then I relapsed. After that I tried to control the inflammation myself, and each time it starts swelling I go to the doctor. I saw ten doctors in almost two years and I m desperate at this point. I don t know what to do anymore. I suffered for almost two years and no doctor seems to have a solution. They all focus only on the symptoms and not on the causes. I went to a new ob gyn this week (hello swelling and abscess again)... at least he listened to me and studied my medical history for more than an hour. It gave me hope again but he couldn t confirm if I have another fistula or not. I asked him about having an MRI to detect the fistula, but he told me it wouldn t be very effective on a place like that. He also asked me if the abscess always appears a few days before my periods. Which I confirmed. What do you think about my case ? Could it be a case of pilonidal cyst in the pubis ? Is it possible to have an MRI on the pubic area ? Thank you a lot !

A pilonidal cyst is an abscess or boil. Treatment may include antibiotics, hot compresses and topical treatment with depilatory creams. In more severe cases it needs to be drained or lanced, to heal. Like other boils, it does not get better with antibiotics.
