hi.. thanks for writing...i have understood your concerns,...
the
paranoid schizophrenia is treated with potent antipsychotics in a high dosage..your wife is receiving the best treatment but the dose need to be increased..
i am suggesting here the treatment strategy..
the serenace can be given in a higher dosage along with a
anticholinergic medication..for sllep problems u shud try sleeping pills..u must consult your treating
psychiatrist and give the prescribed medications..
if with these medications she is not improving then u can take her to the AIIMS and she must be evaluated
resistant schizophrenia.. if the diagnosis is confirmed then she shud be given antipsychotic
clozapine under the observation of psychiatrist...u shud get admitted her first..clozapine is the only antipsychotic approved for resistant schizophrenia...
if with this medications also she does not show improvement then u can go for
electroconvulsive therapy [ECT]..it is very effective and safe,..
for schizophrenia it has been said that 33% will have one episode and then they improve treatment and they may not have any further problem and they can live life normally..
another 33% will have multiple episodes, but with treatment they improve and can become normal between the episodes..
rest 34% will have
chronic schizophrenia, they can be treated but the complete improvement can not be achieved.. they shud take medication life long..
i hope u ll find this advice helpful..best luck..
