Suggest Opinion On Taking Full Dose Of Thyroid Medicine
Tsh 0.066. Had Hashimotos 40 years have been taking thyroxine all that time. Endo reduced my meds, became very hypo but no change to tsh. Had thyroid scan. Thyroid small amount of soft tissue only. Have been like this since aspergillosis in 2011. GP put me back on full dose of thyroid meds and says she is not worried but I feel non-specifically unwell.
Thank you for your query. I can understand your concerns. Autoimmune hypothyroidism (Hashimoto's thyroiditis) at the later stages of the disease may have minimal residual thyroid tissue (atrophic thyroiditis) as in your case. If there is no residual thyroid function, the daily replacement dose of levothyroxine is usually 1.6 g/kg body weight (typically 100–150 g).
Thyroxine replacement in Hasimoto's is life-long.
The dose is adjusted on the basis of TSH levels, with the goal of treatment being a normal TSH, ideally in the lower half of the reference range.
It is important to ensure ongoing adherence, however, as patients do not feel any symptomatic difference after missing a few doses of levothyroxine, and this sometimes leads to self-discontinuation
