Suffering From Anal Fissure Over A Year. Had Bottox Procedure, No Improvement. Suggestions?

I have been dealing with annal fissure for a year now. Had Bottox procedure in October 2012 didnt heal and have episode every two months. Channged my diet always making sure eating enough vegetable and fruit and taking Miralax and when it get worst taking senna and miralax. I am tired of this but scared to do the surgery. Please tell me what can I do I just want to go back to my regular life.

Thanks for the query to H.C.M. Forum.
I know the results of Botox injections are not effective in fissures ( anal fissure).
Conservative treatment ( without surgery ) is there but before this we should know the exact position of fissure and its depth .
2nd point is , if there is pain or not ?
Diabetic or not ?
Hyper tension or not ?
Age & gender of patient .
