Spotting During Pregnancy, Anemic, Braxton Hicks. Help

My granddaughter is here with us this weekend and is in her 27th week of pregnancy, this is her first pregnancy and she was having what I believe is Braxton Hicks....she also states she had some spotting when she wiped...she also knows that she is anemic. This was nearly an hour ago and no sx since that episode. She is a very small framed little gal and very attentive to this pregnancy...no pain or Braxton since that episode. I did inquire her water intake and has not been drinking a lot of water these past few days. Please advise. I am a Registered Nurse, however Labor Delivery and OBGYN is not my area of expertise at this time.

Hi,

I understand your concern and am happy that you are reporting it so early on.
The common causes of bleeding during pregnancy are:
1. cervical ectropion that causes the cervix to pout so that it becomes a bit more friable and bleeds slightly.
2. Show and preterm labor: this is blood stained discharge that occurs as the cervix dilates due to contractions and if not stopped will end in delivery. So one needs to be cautious about this.
So first you need to know if the contractions are braxton hicks.
If you suspect Braxton Hicks contractions, they must be painless, occur at irregular interval and do not have any pattern and there must not be any discharge.
My suggestion is:
Ask your granddaughter to take absolute bed rest that is she must get up only to use the restroom, as this is one of the most effective treatment for preterm labor. Drink plenty of water. Aviod sex. Check for the contractions and note their pattern and duration for which they last. Check to see for any more blood stained discharge.Look for the movements of the baby while lying down and concentrating only on the baby for one complete hour. If there are 3 or more movements its fine.

If the symptoms decrease in 3-4 hours of rest, continue rest and schedule an early appointment with your obstetrician early as she may need additional drugs like progesterone or uterine relaxants to prevent further episodes.

If a contraction occurs for more than 30-40 seconds, and come more than 3-4 in an hour, or if clear fluid comes out of the vagina or there is any further bleeding, see your obstetrician or midwife urgently.
